Very rarely, a CoD4 server does not quit properly and will prompt you if you want to run it in safe mode. If your servers run as a service or you do not have access to the GUI then you will not see this and the server will never start. The only way to fix it that I know of is to log in, run the command line, and click No for the Safe mode.

This is not a gamecreate problem, but I have not found a solution anywhere for this.
